The Health Settlement Navigator (HSN) will support English speaking health care providers create an integrated system of care for the newcomer clients by coordinating a responsive, equitable and culturally safe network of health services. HSN’s objective is to support newcomers and refugees to be successful as they settle in our community by increasing health services accessibility and exploring innovative system responses for health and mental health providers.

Key responsibilities include:
  • Conduct regular pre-arranged consultations to support clients and/or their family’s health needs, taking into account cultural and linguistic preferences.
  • Develop and manage individualized coordinated care plans (CCPs).
  • Provide information about health care services available in London and Middlesex County and support clients along the entire care journey.
  • Identify client’s natural supports and offer additional social and community connections to diminish isolation.
  • Develop, maintain and facilitate excellent working relations with appropriate healthcare and other support services and seek out supports or resources to address any gaps identified.
  • When appropriate, coordinate and facilitate a care conference to re-assess client’s needs.
  • Perform follow-up services to ensure ongoing care and support and at time of transition to another provider ensure seamless handover of CCPs.
  • Collaborate, support and report to the project’s multi-disciplinary advisory committee about progress.
  • Manage and evaluate client satisfaction and other qualitative and quantitative outcome measurements.
  • Be the safe place for clients, family, and caregivers to express their concerns, improvements and/or opinions.
By ensuring the support of and working with a wide variety of health and mental health care organizations and professionals, the HSN aims to develop a wraparound network for services and supports for continuing care.

We are seeking an energetic, engaged, and self motivated individual with the following qualifications:
High proficiency in Arabic and English.
Successful completion a Bachelor's degree in a health related field. 

Excellent computer skills and data management competencies including social networking management.
Demonstrated knowledge of local and regional health and mental health care systems.
Demonstrated facilitation and consensus building skills.
Demonstrated project management experience. 
Demonstrated problem solving and decision-making skills. 
Demonstrated advanced interpersonal and communication skills, both written and verbal.
Demonstrated knowledge of and commitment to the principles of client and family centred care. 
Demonstrated knowledge of and commitment to trauma and violence informed care.

Eligible for this position are internationally trained professionals with at least two years related work experience. The language proficiency has to be documented by a CLB assessment – level 9 in English.
